Prescription Home Delivery: The Channel Matters
Express Scripts' Select Home Delivery
Express Scripts has been committed to assisting plan sponsors with lowering cost increases, in part by controlling and eliminating waste from the pharmacy benefit. One of the areas of focus where waste has been identified is the channel through which prescriptions are filled.
As a valuable tool in driving out waste, Select Home Delivery is one of the many home delivery programs that Express Scripts offers that has been successful in this regard. The program's success is marked by true waste reduction, while maintaining high levels of member satisfaction.
Based on Express Scripts' understanding of behavioral economics and buyer decision making, the Select Home Delivery program relies on the fact that, given an active decision to participate in a program or not, participation increases without impact to satisfaction levels.
The program targets members who are taking maintenance medication, through a comprehensive communications platform. These communications include both pre- and post-implementation letters, as well as automated outbound notification calls designed to introduce members to the program. These communications ascertain whether the member wishes to opt out of the program and remain with the retail option.
Members may obtain their first and second fills of each maintenance medication at a participating pharmacy. If the member chooses to attempt a third fill, without contacting Express Scripts with their decision to remain with the retail option, then a retail intervention occurs. Members are alerted that they must contact Express Scripts prior to receiving their third fill. When the member contacts Express Scripts, they are educated on their options and are then asked
to decide between home delivery from Express Scripts or retail.
Regardless of the decision made by the member, they receive their medication with no penalty.
Since the introduction of the Select Home Delivery program, 50% of targeted members have chosen to move to home delivery. These results were realized without a change in copayments or plan design. With new features, including more payment options, auto refills and auto renewal being added, member engagement and satisfaction are expected to increase.
